Before we can send your disputes, you must fax (include a cover page with contact info) or email us a legible copy of at least two of the following forms of identification:
- driver’s license
- federal/state ID
- social security card
- recent utility bill with current address
- phone bill (not a cell phone bill)
- gas bill
- electric bill
- water bill
- auto insurance
